Clinical Assessment: First Visit

$360-425 (~60 min visit). Initial visits require time to gather the essential information for me to conduct a holistic and extensive clinical assessment. At this first visit, we will discuss your rights to privacy, my clinic policies, and begin to develop a plan to achieve mental health that is right for you. I provide a treatment plan after seeing each patient that outlines steps to move forward with recommendations for medication(s), supplement(s), solution focused interventions, laboratory and diagnostic testing, and lifestyle and behavioral changes. Outside of your visit time, Sue will develop a treatment plan that is unique to you and that can accessed in your patient portal.

Follow-up Visits

Follow-up visits (15, 30 or 50 mins visits) range from $150-300 depending on the timeframe and complexity of the visit. Additional fees depend on added services such as counseling.

Auricular Acupunture

$35 per session (45 min visits). Please inquire about auricular acupuncture packages.

Ketamine Treatment: nasal Spravato/Esketamine

Accepted Insurance Plans

Blue Cross Blue Shield, Cigna, Mountain Health Coop, Wyoming and Idaho Medicaid and Medicare, Select Health, Workman’s Compensation, and Tricare. We also accept Aetna and United Healthcare; however, some of these plans are out of network. Please check with your plan to be sure we are in network. If you do not see your insurance carrier listed here or have any question, please reach out to our office.

We offer a superbill for clients who have insurance coverage from companies that we do not accept. Please ask about getting a superbill if you would like to submit one to your insurance company. Payment is needed in full for any services prior to submitting a superbill to your insurance company.

We strive to be as transparent as possible about the cost of our services.

Please call the clinic to speak with our biller or send us a portal message with any questions about your bill or payment.

We cannot guarantee that lab costs are covered by your insurance. It is the patient’s responsibility to contact their insurance company prior to getting labs drawn. We can provide you with lab order codes to help navigate this process.

Common Questions About Billing:

Is the patient responsible for sending their claim to their insurance company? No, the patient does not need to send the claim to their insurance company if their insurance plan is accepted at our clinic. Instead we send each claim to the insurance company after each visit. Time for claims to process varies and may take 6+ months in some cases.

For individuals who have insurance that we do not accept, we offer a “superbill” that the patient sends to their insurance company. Note that some insurance companies do not cover services that are out of network even when a superbill is submitted. Patients must pay for services in full prior to the submission of a superbill.

Why do rates of service differ between visits? Why are rates at Lupine Mental Health different from those charged by my therapist? Rates for each visit vary depending on the complexity of the services offered as well as the duration of the service. Rates also may differ from those billed by therapists when multiple disorders are addressed, labs are ordered or reviewed, and/or medications are prescribed. Please let us know if you have any questions about your bill or the rates of our services.

Why do you need my credit card to book an appointment? We require that a credit card or debit card is on file for all patients for timely billing and payment. All credit card information is encrypted in our system. Please call us you have any concers or if you do not have a credit card.

How do I cancel my appointment and avoid a fee? We understand that life happens and we all miss appointments for various reasons. Because we often have patients who are on our waitlist and need care, we ask that if you cannot make your appointment to call the clinic so that we cancel your appointment and offer your visit time to waitlisted patients. Appointments cancelled within 24 hours of your appointment time or no shows may be charged a fee.
